New Mexico:
The state Public Education Department contracts with the Northeast Regional Education Cooperative in partnership with New Mexico Highlands University. NEREC is one of nine regional centers in the state that administer IDEA funds and provide services to local schools.
Wisconsin:
The State Department of Education granted a competitive discretionary grant to CESA 11 (Cooperataive Educational Services Agency) to develop and implement the Wisconsin Post High School Outcomes Survey.
Indiana:
The Indiana Division of Exceptional Learners contracts with Ball State University to develop the Post School Follow Up System, analyze results, and provide training. Local Education Agency staff organize and facilitate data collection through staff phone survey methods and data base input utilizing software managed by DOE.
Montana:
LEA's collect the data
Delaware:
The state collects the data from a combination of sources. Over half of the LEAs conduct telephone surveys. The state has contracted with a private organization to collect the data for the remaining LEAs.
Arizona:
The SEA has developed the methodology, forms and web-based process for the Public Education Agency to actually collect the data
Illinois:
Local Education Agencies (LEAs) collect these data under the supervision of ISBE.
