Washington: If the student had an agency connection, information on health insurance
New Mexico: Access of services, participation in extra-curricular school sponsored activites.
North Dakota: Average hours per week individual works;hourly wage; Does the individual have health insurance?;if individual could change anything about their high school experience, what would it be?:Did the individual feel prepared for life after high school?; Does the individual receive services for his/her disability?
Wisconsin: contacts with adult service agencies; accommodations/AT used in postsecondary ed.; IEP planning and outcomes; open-ended questions regarding difficulties they have experienced being employed or attending postsecondary ed. as they had planned
New York: Youth who are eligible to return to school are asked if they are enrolled. Youth who dropped out are asked why, and what would have helped then stay in school. There are questions about the awareness of, and access to adult services. If requested, contact information is provided for these agencies/services.
Oklahoma: programs or classes that would have been helpful while in high school, health care and insurance, paying for things, making life decisions, voting status, etc.
Oregon: agency linkages, high school exoperiences that helped or should have been provided
Indiana: We also collect information regarding adult service agency involvement, self advocacy and overall satisfaction.

Pennsylvania: instructional accommodations received in college, agency linkages, SSA and health insurance benefits received, access to transportation
Nebraska: Adult agency involvement, student involvement in the IEP process, specific questions for those students who dropped out of school
Hawaii: Questions address areas that are addressed in the IEP (social, transportation, recreational, services used, perceptions on satisfaction and preparation
