The SEA collection system provides each district with the list of leavers after a verification process.
• Districts are encouraged to facilitate completion of the exit survey by the parent and student at or near the time the student exits. Students and parents of students who drop out are asked to complete a Dropout Supplement Form in addition to the Exit Survey. Exiting students and parents are advised at the time of exit that a Follow-Up Survey will be sent in one year. Some districts coordinate with local colleges to obtain individuals to assist with interviews. One district provides an incentive to the former student or parent for the follow up survey.
Future education plans, specific to the type of education each is planning to pursue; Work realted training obtained during high school; Extracurricular participation; If no future plans for education, ask reasons for not continuing; future living plans; driver's license; IEP information specific to its helpfulness; Ratings for the level of assistance from school staff regarding transition activities; Components of school that best supported transition activities.
Military status; Marital status; Skills training needed more of while in high school; Job specific information (title, wages, hours worked, insurance/benefits); Reasons for not working or going to school; Community/leisure activities; Agency support; Driver's license; Opinions regarding school preparation; Transportation issues

Reciprocity of information
The SEA Transition Coordinator works collaboratively with the IDEA monitoring staff for responsibility of these indicators, as well as district strategic plans; Transition coordinator provides teleconferencing, Transition Discussion Forum, opportunities for district and school staff to discuss issues related to these indicators
The SEA continues to refine its surveys and assistance to districts regarding data collection. Obtaining the one year follow up responses has been our greatest challenge to date (addresses and phone numbers change frequently).
